Mary T. Inc. utilizes a multi-disciplinary treatment team service model to develop a service plan tailored to individual needs and lifestyle choices. We work closely with clinicians, family members, and our team of nursing and direct support professionals to ensure an effective coordination of community-based services. We focus on the care and support needed to assist the people we serve to achieve and maintain their highest level of community independence.
Residential Services
Located throughout the Twin Cities area, Mary T. Inc. offers you structure and a consistent system of support in a warm and friendly environment. Our professional staff use a multi-disciplinary team-based service approach to provide the therapeutic care and support need to help individuals reach the rehabilitation goals.
Independent Living Services
We provide personalized mentoring, coaching, and support to maintain to promote independence. Our Life Skills Coaches encourage personal growth by providing:

Care for Medically Fragile Conditions
Mary T. Inc. Lynbrook provides long term care to individuals in need of around the clock skilled nursing care in a comfortable Brooklyn Park, Minnesota, home. In addition to providing specialized medical support, the individuals who live at Lynbrook are offered opportunities to engage in a variety of activities such as attending concerts, art exhibits, sporting events, and Mary T. Inc. sponsored activities.
Structured Day Activities
Structured activities are available to provide educational, rehabilitative, vocational, social, and independent living skill opportunities throughout the day. Nursing staff provide regular health assessments while our staff ensure access to medical resources to on-going support.
Nursing Support
Mary T. Inc. has been providing skilled nursing services to individuals with disabilities in group homes and in the community since 1976. Our RNs and LPNs bring years of experience and a holistic approach to ensure both comforting and highly skilled healthcare services. Mary T. Inc. is recognized as a leader in providing services to those with disabilities and complex secondary medical conditions.
Service Model
Mary T. Inc. develops individualized multi-disciplinary service teams drawing from a number of Mary T. Inc. professionals including RNs, LPNs, Behavior Analysts, Dieticians, and service coordinators, as well as others from the community such as neuro-psychologists and physical therapists. The team consults on a monthly basis to review progress and plan next steps.
