Hospice care is a personalized program of services that provides medical, emotional, social and spiritual care for patients with a limited life expectancy. With our help, the patient and family can focus on improving the quality of each day. 

Our hospice team is available to help patients and families navigate an unfamiliar process. We help to make this time meaningful and comforting. Whether in a private home, long term care facility or other alternative residence, timely and compassionate care will be provided. Mary T. Hospice has served patients and their families in Anoka, Chisago, Isanti, Kanabec & Hennepin Counties. We have offices in Coon Rapids, MN and North Branch, MN.

Our team approach to direct care
Our interdisciplinary team includes the Primary Care Physician, Medical Director, Social Worker, Registered Nurses, Massage Therapist, Music Therapist, Spiritual Companion, Certified Nursing Assistants and Volunteers.

What makes our program unique
• We can often begin hospice care the same day we are contacted
• We minimize the number of caregivers our patients see 
• A Registered Nurse familiar with each patient is on call 24 hours every day 
• We help to relieve pain and symptom management within 24-48 hours of admission 
• We provide assistance in arranging additional care through Mary T. Home Health 
• Trained hospice volunteers who offer a comforting respite for family caregivers
• Visits from a spiritual counselor, respectful of the family’s faith and beliefs
• Bereavement support for the family and caregivers
• Additional care is available through Mary T. Home Health

Hospice qualifications
• Life expectancy of 6 months or less
• Functional and/or nutritional decline with weight loss in past 6 months 
• Recurring infections  
• Additional support for pain, symptom management and/or shortness of breath 
• Wish to stop aggressive therapies and repeated hospitalizations  
• Goals are geared towards comfort and end-of-life care needs  
• Caregiver/family support to maximize quality of life

The right time for hospice will be different for each person.  

Payment
Costs are covered under the Medicare, Medicaid Hospice Benefits as well as most private
insurance plans.
 
Mary T. Hospice Care is Medicare Certified and has achieved Deemed Status from CHAP Accreditation.
In most cases, our services are entirely covered by Medicare.

Mary T. Inc. Hospice has teamed with We Honor Veterans to help provide care and support that honor the contributions made by our nation's Veterans. As a We Honor Veterans partner, Mary T. Inc. Hospice will implement ongoing Veteran centered education for staff and volunteers to improve the care given to our Veterans. The We Honor Veterans program was developed by the National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs. We Honor Veterans focuses on respectful inquiry, compassionate listening and grateful acknowledgement.

Hospice care is a personalized program of palliative care and supportive services that provides medical, emotional, social, and spiritual support. Mary T. Inc. Hospice brings care to where the patient lives, enhancing the comfort and quality of life for the patient and their family.

By recognizing the unique needs of American Veterans approaching the end of life, Mary T. Inc. Hospice is able to guide Veterans and their families through this difficult time. In cases where there may be specific needs related to military service, combat experience, or traumatic events, Mary T. Inc. Hospice will work with the We Honor Veterans program to find the appropriate solution.

The nation is seeing many World War II and Korean War Veterans pass away each year, along with a growing number of Vietnam Veterans. In fact, approximately 25 percent of those who die each year in the U.S. are Veterans. Hospice care is a personalized program of palliative care and supportive services that provides medical, emotional, social, and spiritual care for patients with a limited life expectancy. Mary T. Inc. Hospice brings care to where the patient lives, enhancing the comfort and quality of life for the patient and their family.
